-Project: I am conducting my practicum hours with the Clinical Nursing Director at a county community health center. The project I am purposing is to help improve patient quality initiative. This project will aim to improve communication, and documentation.
Be sure to address:
– site / organization: Hubert Humphrey Health is a (clinic) servicing patients in South LA and surrounding communities.
– needs assessment results (or how you knew of the need for your project): Missed doctors’ appointments with inner city and elderly patients and poor patient satisfaction surveys.
– project topic: Providing transportation to local and elderly patients will decrease missed doctor appointments and improve patient satisfaction surveys.
– project goals: To improve quality by making sure patients get to and from scheduled appointments and improving patient satisfaction. Making sure that patients are making their regular doctor visits will also help patients keep up on their health with current treatments and preventative care.
– project outcome: Stakeholders (i.e. physicians, Medi-Cal, Medicare, patients, etc.) will continue to promote and utilize the transportation services provided for appointments at HHH and we will improve our patient satisfaction surveys.
How was your project received by stakeholders? Stakeholders have given positive feedback to the idea and implementation of the project. It has been discussed what transportation services we can use, what patients qualify for the service, and getting insurance to cover the cost of the transportation service to and from scheduled appointments (no out of pocket cost to patients).
If you implemented your project, how will it be sustained? If it is not sustainable, how will the one-time implementation make a difference? Through staff communication and collaborating with insurance carriers, and having reliable transportation services as a part of the program, etc. (Please elaborate from there)
If you did not implement your project, how did you perform a hand-off for the next steps? This project has been taken over and ran by the quality improvement team. They will make sure that are contacts are made (with insurance and transportation companies), the database is updated to arrange for the services, and that nurses and other staff apart of the patients care are able to schedule rides for qualified patients. Patients are also able to request transportation services through Health Net, L.A. Care. (Please elaborate on the importance of a QI team, etc.)
Ultimately, what did you learn from this project? The importance of patient satisfaction, servant leadership, teamwork, and the facilities organizational culture. (Please elaborate)
